  • Health-Integrated Housing Development & Management:

    • We facilitate the creation and management of housing units specifically designed to embed healthcare services within the residential environment. This includes:

    • Pre-Development Activities: Conducting comprehensive feasibility studies, market analysis, environmental impact assessments, site planning, and architectural design.

    • Financial Structuring: Developing innovative legal and financial frameworks that incorporate Medicaid/CalAIM reimbursement dollars and Managed Care Plan (MCO) reinvestment dollars into supportive housing operations and new housing development.

    • Sustainable Infrastructure: Planning for energy-efficient construction, water management, waste systems, and transit-oriented development (TOD) to ensure long-term environmental and operational sustainability.

  • Comprehensive Health & Social Support Services:Our model ensures residents receive continuous and integrated care. Services are often reimbursable under initiatives like California's CalAIM, leveraging Enhanced Care Management (ECM) and Community Supports. These include:

    • Housing Transition Navigation Services: Assisting individuals in securing housing, including tenant screenings, developing individualized housing support plans, searching for housing options, and assisting with applications and documentation.

    • Housing Deposits: Providing resources to cover security deposits, moving costs, and other one-time expenses required to secure housing.

    • Housing Tenancy and Sustaining Services: Offering ongoing support to maintain stable housing, including early identification and intervention for behaviors that may jeopardize tenancy, education on tenant/landlord rights, dispute resolution, eviction prevention, and assistance with annual housing recertification.

    • Care Coordination Teams: Incorporating community health workers, social workers, and behavioral health specialists to provide holistic support, ensuring medically tailored housing and long-term care management.

    • Day Habilitation: Providing services that support daily living and personal development.

    • Medically Tailored Meals: Addressing nutritional needs as a key determinant of health.
